A Case of Suprasellar Germinoma

Abstract
Tumors histologically identical to the seminomas and dysgerminomas of the gonads occur intracranially and are commonly known as "atypical teratoma" or germinoma. The suprasellar germinomas or so called "ectopic pinealomas" were first separated from pineal germinomas as a distinct clinical entity by Kageyama and Belsky in 1961.
We have experienced a case of suprasellar tumor which was confirmed as germinoma histologically after biopsy of the tumor.
A 21-year-old man was admitted to St. Mary¢¥s Hospital in October 1974, because of headache, vomiting and progressive visual impairment for about 3 months.
Clinical examination revealed bilateral visual impairment and papilledema without visual field defect. The urinary specific gravity was fluctuating with tendency of lower level.
Plain skull showed an erosion of the dorsum sellae. Pneumocisternogram showed a filling defect between interpeduncular cistern and prechiasmatic cistern and Conray ventriculogram showed a filling defect of anterior-inferior portion of the 3rd ventricle with dilatation of the lateral ventricle but posterior portion of the 3rd ventricl- showed normal contour without blocking of c. s. f. pathway to the aqueduct of sylviu&
Biopsy of the suprachiasma mass was performed through right frontal craniotomy, and microscopic examination of the specimen showed two cell types, one large and epitheloid, the other small round lymphocytic cells, with the characteristic microscopic patterns of germinoma.
A transient mental confusion was manifested postoperatively but subsided later and discharged without complications. Patient is receiving irradiation therady.
